General Health Office Rules / Guidelines
New Mexico law requires all students be currently immunized or in process before entrance into school and proof will be required at registration. Permission forms to have immunizations given at the SBHC are available from our office.
The school nurse should be informed about any health concern, medical diagnosis or medications a student is taking. Students need a pass or a signed agenda to be seen. Any student going home ill is to be seen and be signed out from the Health Office. Students will only be released to a person listed on their emergency card. Parent consent is required for students to leave campus alone.
Prescription and over the counter medications can be stored in the health office, both require signed forms. Students with parent permission (signed form) may carry a single dose of medications or their asthma inhaler. Medication forms can be obtained on line from the APS website: www.aps.edu , click ”APS Departments”, select Health/Mental Health Services On their webpage, click on “units”, select Nursing “web site” tab. Select “forms” and print needed forms. Forms are also available from the Health Office. Completed forms can be faxed to 848-6432.
Health and Wellness Committee
The Health Office and the SBHC partner with faculty and staff to improve health at AHS by participating in the APS School Wellness Plan. This committee meets monthly and we encourage interested parents, students, and teachers to join.
